Keyworker Team BNSSG (Bristol, North Somerset and South Gloucestershire)

A Keyworker from the Keyworker team at Bristol, North Somerset and South Gloucestershire Integrated Care Board (Homepage - NHS BNSSG ICB) will be allocated to support individuals who have a learning disability and/or autism and have been identified as high need on the CAMHS Dynamic Support Register.

The role of a Keyworker is to be a single point of contact for you and your family to help you navigate and identify the services that you are entitled to. A Keyworker will ensure that you are engaged in the decision-making process regarding your care and that your voice is heard and listened to by all professionals. A Keyworker will not replace or replicate any other service that you should receive, but will help you and your family to break down the barriers you may face when asking for access to the support and care you need.

The aim of the Keyworker team is to make you feel safe and happy and helping you to achieve your goals and ambitions.
