Preparing for adulthood team
The purpose of the Preparing for Adulthood (PFA) team, formally known as the Transitions team is to help facilitate and support young people/young adults, their families, stakeholders, providers and communities to find solutions that enable young people and young adults to continue to live at home or independently within their community.
The PFA team works from year 9 with 14 up to 25 year olds.
The team sits within Integrated Children’s Services in the 0-25 social care service, consisting of a 0-18 disability team and an adults’ team 18-25 with a smaller Occupational Therapy Service covering the teams within the 0-25 social care service.
The PFA team have produced some PFA SEND booklets to explain the various areas of Preparing for adulthood.
