Education, Health and Care plan coordination team
The Education, health and care coordination team, (formally 0-25 SEND Education Team) has a statutory role within the Local Authority.
Its purpose is to coordinate statutory processes under The Children & Families Act 2014 and The Special Educational Needs and Disability Code of Practice 2015, and other associated legislative frameworks. This includes making decisions on EHC needs assessments, issuing EHC plans and ensuring annual reviews take place so that monitoring and planning focus on the child or young person’s needs and progress.

To access the 0-25 education service you will need to have an EHCP needs assessment.
You will need to request an Education Health and Care needs assessment.
Requests will be through council's the Access and Response Team (ART)
