Preparing for adulthood SEND guidance

Young people entering into post-16 education and training should be accessing provision which supports them to build on their achievements at school and which helps them progress towards adulthood. Young people with EHC plans are likely to need more tailored post-16 pathways. SEND Code of practice section 8.20.

South Gloucestershire has a 0-25 Preparing for adulthood team ( formally known as 0-25 Transitions team that helps facilitate and support young people, young adults and their families, partners, providers and communities to find solutions that enable them to continue to live at home or independently within the community. 

Preparing for Adulthood Pathway Guides  

The team have produced preparing for adulthood pathway guides that can be used by education settings, professionals and families to help young people think about planning and preparing for their future.

The guides will be updated on an annual basis. Over time the organisations listed within the guides will be available on the online directory which you will be able to search in.
